I understand that Med DataLink GI provides a thorough and complete review of the data submitted for evaluation — marking all pathology, key landmarks, images, special findings, and other critical parameters as necessary — and generates a detailed data analysis. The referring physician interprets these findings, makes the diagnosis, finalizes, and signs the data analysis.

I understand that payment is due upon receipt of invoice. If payment is not received within 7 days of invoice due date, late fees and finance charges will be assessed.
